A few notes before you begin ...
- Mixed Elections This Plan Year, you will have the opportunity to select varying levels of coverage for you and your family, to build a more personalized health plan profile (i.e., Employee Only for medical, Employee + Children for dental, and perhaps Employee + Family for Vision). The Online Enrollment Form will display varying levels of coverage based on the dependents you may have listed, and you will have the option to mix your enrollment to suit the needs of yourself and your family.
- Dependent Detail The Online Enrollment Form may auto-populate some spouse data if we have it on file. If you do not see your dependents populated when you access the Online Form, please add them when prompted.
- Health Incentive Rates The Online Enrollment Form will show appropriate rates based on your region and Health Assessment status. If you have not completed your Biometric Screening and completed the online Health Assessment yet, don’t worry; you will have until the end of June to complete this and we will verify the status of all Health Assessments before implementing the new rates for 7/1.
